We aim to make your care as convenient as possible!
NEW PATIENTS / FIRST OFFICE VISIT
At the time of your first visit, please bring your health insurance information/ID card, doctor’s prescription, primary care physician referral, driver’s license, and other information that might be helpful in diagnosing or treating your problem. Plan to arrive at least (10) minutes prior to your scheduled appointment time in order to complete the necessary forms.
APPOINTMENTS
While we try to keep waiting time to a minimum, emergencies may occasionally disrupt our schedule. If a significant delay in the therapists’ schedule occurs, the receptionist will try to notify you so that you will have the option of rescheduling your appointment, or waiting until the therapist is available. If you are unable to keep a scheduled appointment, please notify us at least twenty-four (24) hours in advance so that we may accommodate other patients who may need care.
If you are more than (15) minutes late for a scheduled appointment it may be necessary to reschedule you, so as not to inconvenience the other patients.
There is a $25 fee charged for all NO SHOW / NO CALL visits as well as Same Day Appointment Cancellations.
REFERRALS / AUTHORIZATIONS
Depending upon the requirements of your insurance plan, a referral from your primary care physician may be necessary in order to see our therapists. If a referral is required, it must be presented before you will be able to see the therapist. This is a requirement established by your insurance plan that we must follow in order to be reimbursed for the services our therapists deliver.
