For your convenience, all of our clinics offer same day appointments. Please call to check on availability.
Before I schedule my first visit
What do you need to do before you schedule your first appointment with us? It's simple.

1. You may see a physical therapist at Advanced Center for Physical Therapy without a referral or prescription. However, some insurance plans will not pay for physical therapy without a written PT referral. Contact your medical provider, such as your doctor, chiropractor, podiatrist, dentist, osteopath, or specialist, and ask for a written referral to physical therapy. Your primary medical practitioner can fax a prescription to us especially if they have seen you for a condition in the past. Your primary medical practitioner may want to see you for an appointment to evaluate your disorder first before they give you a written referral to physical therapy.

Insurance companies generally accept prescriptions from physicians, osteopathic practitioners, podiatrists, naturopaths. Non-physician practitioners include physician assistants (PA-C), and nurses. In some cases dentists and chiropractors may refer.

2. Once you have the prescription or referral for PT, call our clinic most conveniently located for you. You will be asked questions regarding your insurance coverage. Have your insurance card in front of you. You will need:

• Your ID and group number,
• Your birth date and that of the subscriber's, if not yourself,
• An insurance customer service phone number so that we may call them to verify benefits. We do this as a service for our clients to help you with insurance coverage for physical therapy services.

3. Next, an initial evaluation appointment time can be established. We will do our best to accommodate your scheduling needs. All of our clinics have early morning and early evening hours to assist in scheduling ease. These particular times of day are in high demand, so please be prepared to be flexible for your initial evaluation, which takes longer. Plan to be in the clinic an hour to an hour and a half for your first visit. Subsequent visits should take between 45 minutes to an hour. Click "About Your First Visit" for details about what to expect when you first visit one of our locations. For more information regarding our services, please contact the Advanced Center for Physical Therapy location most convenient to you. It will be our pleasure to assist you.
About My First Visit
When you come to the clinic, we will need you to bring:

• Your insurance card
• The referral/prescription for PT
• Any applicable co-pay

To ensure accuracy of your records, please keep us informed of any changes that may occur such as: insurance coverage, change in address, change in physician, or any other pertinent information.

During the initial evaluation, the physical therapist will examine you and will establish a plan or care for further treatment and begin a home exercise program. Our therapists are trained in manual therapy techniques to assist in your recovery from injury or dysfunction.

Your therapist will develop a treatment plan that will include your own personal goals, as well as goals established by the therapist. These goals will be reviewed on a regular basis to track progress and improvement in your program. The evaluation will take approximately 60 minutes.
